发病时间

典型症状
痛风会呈现出许多种症状,最常见的是急剧的刺激性关节炎(关节红,软,热和浮肿)周期性地发作,而在脚拇趾基部的跖趾关节最常受影响,占一半的病例,原因可能是因为脚趾温度较低,加上脚趾附近的液体会被身体吸收使尿酸浓度增加所造成。其他关节,例如脚踝,膝盖,手腕和手指也有可能受影响。关节疼痛通常在夜间发生且持续2到4个小时,这有可能是由于体温较低而产生。晚上睡觉时,人体的抗炎物质促肾上腺皮质激素在半夜时分泌最少,加上睡眠时酸碱值因体内累积二氧化碳而偏向酸性,因此痛风最易在半夜出现。其他的罕见症状包含疲倦和高烧也有可能伴随关节疼痛发生。 认知人类基因
治疗方法
原发性痛风目前尚无根治方法,但控制高尿酸血症可使病情逆转。治疗目标主要为缓解急性痛风的症状。

- 一般处理: 蛋白质摄入量应限制在1g/(kg·d)左右。不食用高嘌呤食物(动物心、肝、肾,沙丁鱼等),严格戒酒,避免诱发因素。鼓励多饮水,同时,不应使用抑制尿酸排泄的药物,如氢氯噻嗪(双氢克尿噻)、呋塞米等。
- 急性关节炎期的治疗:应绝对卧床休息,抬高患肢,避免受累关节负重,持续至关节疼痛缓解后72h左右方可逐渐活动。应尽早应用非类固醇消炎止痛药(NSAIDs)、秋水仙碱与类固醇等药物控制关节炎,缓解症状。
- 间歇及慢性期的治疗:虽经上述治疗,但症状仍不易控制、反复发作者,可用小剂量秋水仙碱维持治疗,在用药过程中应密切注意秋水仙碱对骨髓的可能抑制作用和定期复查肝、肾功能。合理应用具有抑制尿酸合成与促进尿酸排泄的药物,控制高尿酸血症。
- 并发急性肾衰竭的治疗:由尿酸性肾病所致痛风者,应立即给予乙酰唑胺,同时静脉补充足够的水分,增加尿量。血尿素氮和肌酐升高显著者,可行血液透析或腹膜透析。

- 可控因素
- 饮酒:酒精饮料中含有嘌呤,会增加引起痛风的风险。其中啤酒和黄酒的嘌呤含量远远高于其他酒精饮料,相对于其他酒类有更高的致病风险。
- 饮食习惯:高嘌呤食物会导致患痛风风险升高,高嘌呤食物包括动物内脏、海鲜鱼卵、浓汤肉汁等。近来还有研究指出高糖(如碳酸饮料及果糖)也会导致痛风。而维生素C,咖啡和奶制品则可能对阻止痛风的发生有所帮助。
- 肥胖:肥胖会导致尿酸浓度上升,诱发痛风。
- 运动:运动可以促进尿酸排泄,但剧烈运动却会导致尿酸浓度上升,增加患痛风的风险。
- 尿酸浓度:体内水分缺乏会导致尿酸浓度过高,增加患痛风的风险。
- 精神因素:精神压力增大及疲劳过度都也可能导致内分泌紊乱,造成尿酸的代谢异常,使尿酸浓度上升,诱发痛风。
- 低温环境: 尿酸盐在温度低的环境中更容易析出为晶体,沉积在关节周围,引起关节炎发作。这也是痛风容易在夜间发作的原因之一。
- 不可控因素
- 年龄:痛风一般在40岁之后发病,且随着年龄增加,患痛风的风险也会上升。
- 性别:女性一般在50岁之前不会发生痛风,因为雌激素对尿酸的形成有抑制作用;但是在更年期后会发病率增加。
- 家族病史:有痛风家族病史的人,患病率较高。
- 其他疾病:肾脏疾病或某些药物所致尿酸排泄减少、骨髓增生性疾病及肿瘤化疗所致尿酸生成增多等,有可能诱发痛风。
